Support Coordination

Support Coordination helps individuals with disabilities to navigate and access the NDIS support services they need.

A Support Coordinator works with individuals to identify their goals and needs, and helps them to develop a plan to achieve those goals. They provide information and guidance about the different types of support services available under the NDIS, and help individuals to connect with and access these services.

The role of the Support Coordinator is to help individuals to coordinate their support services and make sure they are getting the most out of their NDIS plan. They may also work with other service providers, such as healthcare professionals and community organizations, to ensure that individuals are receiving comprehensive and coordinated support.

Universal Ability offers support coordination services to NDIS participants. Our support coordinators work with participants to develop and implement their NDIS plans, connect them with service providers, and coordinate the delivery of support. We can also provide advice and information about the NDIS, assist with complaints and reviews, and advocate on behalf of participants. Our support coordination services are tailored to each individual’s needs, and we can provide as much or as little support as required.

NDIS support coordinator at Universal Ability will work with NDIS participants to develop their NDIS plans and connect them with the services and providers they need.

They will also provide ongoing support to NDIS participants, monitoring their progress and making sure they are receiving the best possible care and support.

NDIS support coordinators at Universal Ability will also liaise with NDIS providers to ensure that NDIS participants are receiving the services they need and that these services are of high quality.

They will also work with NDIS participants to review their NDIS plans on a regular basis, making sure that they remain relevant and that they are still meeting the participant’s needs.
