Support Coordination helps individuals with disabilities to navigate and access the NDIS support services they need.
A Support Coordinator works with individuals to identify their goals and needs, and helps them to develop a plan to achieve those goals. They provide information and guidance about the different types of support services available under the NDIS, and help individuals to connect with and access these services.
The role of the Support Coordinator is to help individuals to coordinate their support services and make sure they are getting the most out of their NDIS plan. They may also work with other service providers, such as healthcare professionals and community organizations, to ensure that individuals are receiving comprehensive and coordinated support.
